Learning Disabilities service and referral resources

All information you will need to refer a patient to our service can be found at the bottom of this page, under Downloads.

To find out how to refer to our service please use the easy read Learning Disability Service Referral Leaflet.

To find out if your patient is  eligible for our service please download our Eligibility Criteria.

If your partient is eligible and you wish to refer to refer them to our service please, download and complete our Referral/Assessment Form.

If you need further information about the Mental Capacity Act before completing the Referral/Assessment Form you can get more information by downloading the Mental Capacity Act Summary from the Department of Health website through the link at the bottom of this page.
